gotta love getting a mushy pedal and then go to bleed and you see thise on each side. a must pedal due to a caliper pad bolt coming lose on each side. i have never taken them out either so very crazy they would come loose now. thank god the piston didnt come all the way out. this happened on the left and right caliper, same session

We have had this problem many many times. Once it was an outside bolt, when slowed down enough it slid out and ripped some spokes out of a brand new wheel. Removed all the bolts and re applied some super strong locktight to the threads. Keep spare bolts, pistons and pads on hand. Almost ruined a 4 day track event at WGI. Keep a close eye on them.

Not sure what causes them to come loose. Mine were frozen in the caliper and I was afraid I was going to break off my new Torx bit like I did the previous one trying to get the bolts out. Then a grinding noise while on track and my LGM wheel has a groove on the backside of the spokes due to a pin that was frozen in for the last year and a half coming out.
The only good solution may be to get the bolts with regular heads on them and safety wire them.

Another thing that could be done is to clean the heads and the surrounding caliper with Wax and Grease Remover and then use Permatex Ultra Black Gasket material to keep the bolt from turning. Just put a smear on the edge of the bolt crossing over onto the caliper. Don't put it in the area where you would put your Torx wrench. This stuff is good up to 500 degrees and should put just enough stiction on the bolt to keep it from turning. When you want to remove the bolt just take a razor blade and cut the gasket at the edge of the bolt hole.
It should only take a small amount on each bolt head.

I got rid of my padlets the first track event. I even run Carbotech Bobcat one piece pads on the street.
