Coleman 2pc rear rotors, base C6 size, anyone have build sheet?

I need to get these ordered or send them a rotor to mimic in a 2 piece, anyone already done this and can share the build sheet or give me your info so I can reference your order and mimic it?

I may have figured this out: buy them from PerformanceAFX. I just need to confirm with them they use Centric rotors.
