I personally have only ever seen one. It is on a 691 valve I bought some time ago. It does not seem designed to be removed from the valve easily. Maybe over the years the paper has shrunk, I don't know. The point I am making is I do not know if the paper tag was available without the valve.

What are you planning to put it on?

I'm like you I doubt if the tag was available without the valve. I have three NOS 691 valves that look very similar to yours with the exception of that the writing is all on the same line.
Send me your email and I will send photos of the tag I put on my car.

No, I am going to hang on to it. Of all of the parts I have ever looked for this is probably the one thing that has been hardest to find (paper tag portion). Most everything else can found if you are willing to pay the right price. I have continued to look for another one of these and have never seen one. I still have never been able to confirm that it is even the correct part for my car, but I think a judge would have a hard time saying it is not.

If there is any way I could borrow it long enough to copy the tag I would appreciate it.
