Can't accept incoming friend request
Received a 'friend' request from a member that hasn't been active for
a while. (last post was 2/28/2015).
Noticed on their visitor message page in their profile, that others have
experienced the same problem of accepting his friend request.
Saving the 'accept' (default) friend request does nothing and the
notification remains that there is a pending friend request.

I highly doubt it is an account issue. Many members miss putting the required checkmark in the member's avatar, for each request, before using the accept button.
In this image below, next to the green line, you need to put a checkmark in the member's avatar before the accept button. Otherwise nothing happens:
