man this is a chronic problem. ihad one fall out too. i took a small bolt of the right diameter and used heat shrink to build the "shoulder" up around the shank of the bolt. i installed it and double nutted it. its worked fine since. but really i need to replace this. put me in line for one of them please.

Yes they are a pain. I had one that wouldn't come off during a projector install so we had to cut it off.
