I hear ya. The forum is built on a really old tech platform with old User interface design..... this bug you point out is a "feature" in that way. So anyone who is not used to tech, is frustrated, and those of used to new style interfaces, are also frustrated.
the title seems redundant, and hardly anyone uses it. Plus the actual thread title is only editable by an admin. This is why you don't put any info there that might change. Say, like a price on something. God forbid you put in a typo, you can't fix those either.
The "response" title may serve an old tech purpose in a word search, it could get a higher ranking than the text in the messages.
For example, I do a search on the following phrase (with misspelling) and I get a direct hit in this thread.. Quotes around searches help.
"Alziemer checkup".
Back to your first question, the box around the thread number, just a user interface design that came along, probably, a long time ago.
