The 5th gear test is a good one for clutch. 40mph, go wide open throttle in 5th. It should climb slowly in rpm, engine bogged.

A slipping clutch will feel like that, the rpm will jump and spike up, the car doesn't accelerate linearly. When its really gone, the rpm shoots up like crazy without the car accelerating anywhere. It will affect the lower gears in the same way until you can't even launch the car.

I've got 120,000 on mine with no signs of failure yet. The service writer at my local dealer said they generally last until 90,000 miles.
