Per my '05 service manual; HO2S Circuit Insufficient Activity Bank 1 Sensor 2. ("H" heated, "O2" oxygen, "S" sensor). The back O2 sensor on the drivers side is reporting a problem.
So is this a bad 02 sensor. I was told by the dealer that my Kooks high flow cat was bad?
I think its your 02 sensors connectors and not your cats (yet). If you had a bad cats you would get the infamous P0420 / P0430 codes.
Check you 02 connectors, extentions, and wirings (make sure they're not pinched or melted) . Remember you have Four 02 sensors that had to be extended.

just clear the code out. its a low flowing cat code, from the headers and high flow cats it will come on time to time. it doesnt affect performance it just comes up because you are flowing to fast to read or back sensors are turned off. so it thinks your cats are not flowing it must come up after so many cycles. buy a code reader and turn it off when it comes up

If you have LT headers w/cats you will most likely get the 0420 and 0430 codes, they can be "adjusted" by a tune so they don't show. Supposably the cats are still functioning fine even if you get the code, the codes come on because of the distance between the front and back O2 sensors, the back O2 sensors don't get enough heat fast enough.
Thanks for all the help. I'm going to get under there and check my wiring because I do remember a slight smell of plastic burning last week. Also what code reader do you all recommend and where to get it.

I got the "very basic" Innova 3030 for about $70 at Amazon.com. Very happy with it.

Not always true. Remember that Hi-flow cats are a 2 way cats not 3 way like the factory and thus not OBD II complient. The computer will sense it and it will trigger the CEL. Anyone with a High flow cat with or without headers (even if the hi-flow cats are in the same location as the factory) will sooner or later get the P0420/30 if you dont get the 02 sensors tuned out.
