Doesn't have to be the dealer you bought it from for the GM one. Search Fichtner on the forums and you'll see the place I bought mine from.
There are some non-GM ones out there too, I went with the GM one though.

Make sure you read the fine print (especially if it isn't from GM) to make sure you know exactly what it covers and what it doesn't, where you can have repairs done, etc...

Since Michelin runflats are only allowed 1 flat repair/tire and if near the side wall are NOT repairable, well worth the $255 I paid IMO.
One rear tire at Discount Tire or Tirerack prices cost ~$480!
When I checked with a local Chevy Dealer they wanted $1400 for tire/wheel insurance! If I hit a wheel, my bad but these wide tires find nails that I can't control!

I bought mine from the dealer but in 3 1/2 years have not used it. I did a bit of curb rash on the right front wheel but the warranty didn't cover curb rash so I had to pay to get it fixed myself. I don't know if any policy covers curb rash but it is worth checking out.
