I’ve been researching this topic since the 4 got released. From what I can tell, the V4 addresses some rare and random interface issues. The V3 was advertised to potentially not function with 18-19 model years, and V4 does not have that warning, so my guess is that’s the biggest reason for the 4.

Yeah, from what I know it works with 18's but some 18+ cars have performance tire mode issues. I got this one after my first one (V2), had issues and its been reliable. The v4 has a different processor so that will work with the new firmware coming to enable performance tire mode fixes for these cars. Other than that, it uses the same software, it will function identically to the new unit, including PTM.
Let me know if you have any more questions. I'd be cool with letting someone test it, but I'd hate to have someone bend the pins and send it back.
