or give my assistant Albert Bummer a call at 406-628-4618 ext 105
We will email or fax you the transfer form.
Complete the form, attach the policy and ID card and mail it to GMPP with a check for $35 (if the plan was purchased prior to 10/1/02) or $50 (if the plan was purchased on or after 10/1/02). The new owner will receive a new policy and ID card in their name shortly!
