I will probably take it. I have to take clutch and tranny in on Monday and my guy wants to look at all my parts before I buy anything new, but I am pretty sure the fork is bad.
Where the bearing rides on the other side, is that in good shape as well?

I removed it from a car that I converted from a Muncie to a TH400 and I cleaned it up before I threw it into my parts box. I don't know what the history of the part is, other than it was functioning properly when it was removed.
