Default alignment issues-problem

which causes massive inside wear on a tire....toe in or toe out and which is toe out....wheels pointed out or pointed. this wear is with camber as it should be. thanks.

Toe out is ... out.

Massive wear on the inside sounds like a serious camber problem more than toe.

i cannot imagine that too much toe-in/toe-out (toe-out is with tires pointed out) would cause 'massive inside tire wear' without having incorrect camber.

too much toe-in/toe-out with correct camber should cause an even scrubbing of the tire tread off across the entire tire tread.

correct toe-in/toe-out with incorrect camber will cause tire wear inside or outside of the tire tread (depending on whether it's positive or negative camber)

Typically camber due to either poor alignment setting (not likely), or most likely a worn ball joint / kingpin (depending on suspension system) or a spring that has "settled" lower. Understand you say camber is as it should be, but you asked!

As stated by wmf62, toe being really off can cause drastic wear, but it is across the face of the tire, and you normally also see feathering across the face as well.
here is a list of alignment caused issues as listed on Discount Tire web site:

Misalignment Condition = Tire Wear Symptom
Incorrect Camber Setting = Premature smooth wear on either inside or outside shoulder
Incorrect Toe Setting = Feathered wear across tread; raised tread block edges
Incorrect Caster Setting = Excessive shoulder wear; tread blocks show "heel-toe" wear pattern
Unequal Caster setting (either right or left side is out of specification) = Sharp pulling necessitates steering compensation and feathered wear
Unequal Toe setting (either right or left side is out of specification) = Sharp pulling necessitates steering compensation and feathered wear
Combination of two or more settings are out of specification = Irregular treadwear with feathering and smooth spots

And Toe-in is with the tires pointing inward at the front of the vehicle (a small amount is normal for street driving), and provides a stabilizing affect. Toe-out can be used, especially race vehicles where quick turning is required such as in autocross.
