I think the KH print called for the drive pin holes to be .525/dia. The drive pins on the adaptor about .005 less than the wheel holes.
The later re-pop adaptors seem to have smaller pins. Maybe .513/.515.
You picture looks like the wheel was installed wrong at least once.
It's hard to say why one particular wheel came off twice but if I was guessing, I'd guess it wasn't installed correctly.
If your wheel pin holes are round and measure close to what I spec'd above, the threads good and clean on the spinner/adaptor, I don't see any reason not to use what you have.
I run two sets of these wheels. Never had a problem. I don't use the safety pins and I never check them. Not telling you to do as I do. Do what you think is logical to you.
PS. I've bought and sold about 5-6 sets of KO's in the last few years. Some wheels in the drive area looked so bad I wouldn't sell them as part of a set. Yet, I bought them as a set and somebody had been running them. Some of them had come off, too.
I never had problems with mine and I use the "safety pin" in all four wheels. Again you need to put the wheel on CORRECT for it to work..
I hate to be the bearer of bad news, but looking at that photo of the wheel, it is ruined. Once a KO comes off it is usually done depending on how it came off. The wheel and cone have a mating surfaces machined into them and fit together like a bearing and race except it is designed to hold it tight and not spin. When they come off it usually destroys the machined angle in the wheel and they will not hold tight after that. That wheel looks like the machine area is completely gone and the adapter made threads in it as it came off.

Justin, thanks for the input. But, the area seen with the seeming threads cut, are not in the machined area, but in the cast part just inside the machined area. That machined area appears undisturbed. I set all 4 wheels beside each other and those areas all appear OK. Yes, I agree there could be damage that can't be seen. I am looking for a replacement, anyone have a spare? Speaking of spares, maybe this one could be used as a spare. Thanks again for all of the help.
