it means different things to different people....the last part of your description is closer to the accepted definiton....it can also mean over restored. generally the maintenance factor doesn't come into play....they want everything to be near perfect, correct and very clean.
"Concourse" condition may mean that it's sat on the tarmac at O'Hare between the H and K concourse for the past decade. :
"Concours" condition usually means that the car is over-restored to Pebble Beach quality standards. Perfect mirror finish paint and chrome, tight and uniform body gaps, and not a spec of dust or dirt anywhere on the car.

The last judged corvette show I took my car to last year they, the club putting the show on entered my car into the concours class. I always enter the stock class but they felt it should have been in the concours class. Anyway your car has to be like new and correct in all areas.....engine & compartment, undercarriage, wheel wells, exterior, interior, and in most cases it has to be better than when it left the factory. The difference between concours cars usually is very very small. Now whether the folks judging your car know the difference is another matter.
We had two classes one original and one modified. The original line had to look like it just came off the assembly line and the other line had to perfect.The only dirt allowed anywhere was the dirt you picked up driving across the parking lot. Yesterdays dirt was a big deduction. Oil and grease on a motor or tranny got you a trip out of the tent.
It's a lot of work but it feels real good when you collect an award at the end.
