Default water pump

Heh has any one had any problems with water pump bolts coming loose .
has been happening since i bought car ,came loose about 3 times first thought it was water pump have replaced that now ,i suspect clutch fan but cant see any problems but i dont think you would .did have a bit of vibration the other day but went ,i would like to order new clutch fan but thought i would ask here first to confirm it.

Random thoughts
1- please verify that you are using new lock washers when you install the bolts

2-please verify that you are torqueing the bolts

3- are the threads on the bolts or block worn / damaged?

Yes new arp water pump bolts and washers
and yes torqued them correctly
and motor has been rebuilt inc cleaned and tapped all threads
