This is a term applied when you install the distributor with #1 piston at TDC but on the exhaust stroke instead of the compression stroke.

When a distributor is 180 out it means that when your engine is at TDC (Top Dead Center) the rotor is basically pointing to number six on the Distributor Cap instead of pointing to number one. Of course this is all on the compression stroke.

Its based on the fact that the distributor rotates at half the speed of the crankshaft. Therefore when your timing marks are lined up at 0 either the rotor should be under the number 1 plug wire, or 180 degrees away from number 1.
