Ever since I've hooked up my AC on my 74' four-speed last year. I keep throwing the belt if I rev it up over 5k. I've tried a couple of different brands of belt and I tried tightening the heck out of it, risking the water pump bearings and seal, yet I threw it again last night and it sliced my lower radiator hose on the way out. Any ideas what can cause this, or a better way to align the pulleys or something?
Get a straint edge and check the pulley alignment. Most likely, one of the pulleys is not in the same plane as the others. Most likely the AC, since that is when the problem started.
PS. Do not trust your eyes. They may look aligned until the strait edge reveals otherwise.

You can also use one of those little pocket laser pointer pens....lay it in the groove and shoot it at the other pulley and see where it hits. Works well!!
Just for reference...I never have luck with the Dayco Top Cog type staying on...but the Gates ones live well for me at high RPM.
also make sure your pulleys don't have no bends/dinks in them...... that's what was wrong with mine and after throwing 4 belts i tore it all down....... i had 2 belts wrap around my upper rad hose and pull it off....wtf....... spwed coolant everywhere....so not cooll.......hahaha
