Not sure, but I picked up a kit for about $35.00 at Carlisle a few years ago, and had someone else make the swap, I think oils and O rings have to change in the compressor and new fittings, put on your lines, you are supposed to change the dryer and evaporator due to R-12 contamination, but I didn't, and it seems to work fine.
To change, you need to replace the accumulator and orfice tube and at least blow out the lines to get as much old oil out as possible. Put in new oil and charge to about 2/3 the original charge. My experience is mixed with GM R4 compressors-some immediately eat the front seal, others don't. If I was going to do the swap, I'd change the compressor and be done with it. R134 is much more sensitive to charge than R12. You really need gauges to do it right and you absolutely have to vacuum it out before you charge it.
