The most common way a balancer goes bad is the rubber between the inertia ring gets worn and cracked. This will cause the balancer to spin, and the timing mark to be way off. An easy way to inspect the balancer is to look at that rubber ring and see how bad off it is. If it looks cracked then it is probably in your best interest to get a new balancer.

Mine actually came aprt on my way home from Eckler's last year. I hadn't check the timing for a while so I had no warnig. The outer ring just banged and clattered. I was almost home so I just took it real easy.

Without a doubt, it can be difficult to track down unusual noises.
I REALLY thought that a failing harmonic balancer was making noise on my car but it turned out to be cracked crankshaft pulleys. A cracked crank pulley caused my rattling/squeeking noise (pic)http://forums.corvetteforum.com/zerothread?id=438729

just a note...i havent got the stock balancer...
i have one with liquid in it...
does that make a difference?
would it sound or act differently if it was bad?
