HELP!! Is my P/S pump bad?

Last week I heard a squeal and then P/S and A/C belts pop off right after start up. Alt belt stayed on. I fished them out, restarted it and revved it. I still heard a brief sqeak when I revved it, but it sounded more metallic, so I replaced the water pump. I wanted a high flow anyway.
Yesterday on the way home, I got on it from a light and took her up to nearly 5000 rpm. The P/S pump belt popped off, and the A/C belt rolled upside down. They were both brand new, BUT: the PS belt was so tight I had to walk it on and I could not move the pump out AT ALL for adjustment. The A/C belt was as tight as I could make it, but still flexed a lot (actually vibrated like a guitar string) at idle.
Last night I examined the pump: there is at least 1/16" run out on the pulley, and at least 1/16" play in the shaft, in-out and up-down. There is also a shallow groove cut in the cross member below where the pulley rubbed on it. I suspect it was when I had bad motor mounts and the engine leaned over briefly around a turn or on acceleration.
Would your best guess be P/S pump and pulley as the cause for the thrown belts? I hate replacing stuff only to find I still have the problem
1/16" up and down play sounds excessive, check the manual for specs. or stop by the auto supply a feel up their pump. :D
