Reverse rotation engines are used in twin-mill drives. One engine turns clockwise, the other counter. While the main apparent difference is the cam and starter, the cranks are also different. Cranks have sipe scores that ride on the rear main seal that are cut in the direction of the rotation. If you use a reverse rotation crank in a clockwise rotation engine it will seep oil out of the rear main and you'll never figure it out unless you know about this.
