I have been confused on this myself. The assembly manual shows seat belt as standard and the shoulder harness as RPO A85. I haven't been able to confirm if they were retractable. I have a friend that I will see Saturday he is big into NCRS. I will find out from him.
Okay I looked on the 1969 registry and as I mentioned above, std on coupes and only 600 of the 16,633 verts bought the shoulder belt option...That's why you see so many w/o it.
The shoulder belt has a sort of "mushroom" peg that fits in the hole of the lap belt. Then, you slide it in the connected slot and it will "click" in place.
My 69 coupe (late) has the original belts. The shoulder belts are on retractors (the retractor mechanism and belt has a flexible plastic boot covering the unit) and bolt into the rear wheel well as in the picture posted above. They pull over the top of the seats and through a guide that is under the head rest. The latch has a post which locks into a corresponding hole in the lap belt.
The lap belt passes from the door towards the console and has a slider used for adjustment. My memory says that there were several styles of latch mechanism used on 69s. I believe that early models used the same mechanism as the 68. The later 69s (mine) used a smaller latch mechanism and it was mounted in a plastic sheath which held it up at the seat level and prevented the latch from dropping down between the seat and console.

I have an Oct.69 car.. The shoulder belts have retractor and go through the guide on the seat back. The buckles are both metal and the the lap belts store in a clip on the front of the seats. (No lap retractors)
