Put a little fish-tank gravel and some Dawn dish washing detergent in that bottle and shake it around. It will clean up like new. I don't think the inside of the engine will see anything like that because there isn't, [or shouldn't be], any air in the cooling system to cause it to scum up.

I just flush and change it about once a year. In less than a year it begins to gum up on me. A good test for me if its time to flush and change it is to look in the overflow tank after the car has been sitting for a few days, before starting the car. If there is gunk floating on top its time for a change.
