A. The bushings are worn out or ground/crushed into pieces (VERY common).
B. The shaft that goes through the middle of the gear is spinning/slipping inside the gear (this happened to me and is apparently exceptionally rare as I am the only person I have heard this happening to). The shaft has some splines on it to prevent this, but mine still did it anyway. The solution was take it apart and put it back together. When I put it back together, I tapped the shaft through the gear with a sledge hammer and the fit was much, much tighter. Problem solved.
