That hose on my '94 was in worse shape than that. I found a piece in the Dorman section (HELP) of the local Autozone that fit and fixed it right up, seems I may have trimmed a little off. Sorry, it's been a few years back and I don't have the numbers for you.

Not ever attempting to use the "HELP" branded part and I would have guessed that maybe this elbow actually might have had a different ID on either end BUT the 47029 which is a MOPAR PCV elbow replacement might be the referenced part.
It was early 90's and the OE was 4536156 and might be less expensive if it's a fit from the dealer. Maybe?
