The 4+3 trans is a manual transmission. There was a thread on here a few days ago about it where Scorp explained it pretty well, do a search for it.
I don't know what you are talking about with the suspension unless it has to do with the wheels all being standard 17" starting in '88 but '87 had that as an option.
I've always thought the 4+3 sounded cool. I couldn't find one in a good car when I was ready to buy my vette, so I went with an automatic. Looking on the forum over the past few months, I have noticed that the people who have them are, more often than not, complaining. It seems that the rear 'overdrive' component requires a very high level of attention, or it overheats.
Like I said though, I don't have one, but I have been looking at them.

The Doug Nash 4 speed is a good transmission. It’s the Overdrive unit that is the weak link in the 4+3. You have to take very good care of it, (change oil & filter regularly like Scorp said). Also don’t drive it like it says in the car’s manual, it was written by GM to get around EPA gas guzzler laws so they tell you to drive with the Overdrive engaged to save gas, but that will kill the Overdrive unit prematurely. The actual manufacturer of the Overdrive unit says to ignore GM and never use the Overdrive unit unless going faster than 50 mph for extended periods. Don’t drive around town with it engaged and always disengage it on startup as not to forget and drive around with it engaged. You will get a small fraction less gas mileage but your Overdrive unit will last a LOT longer. :yesnod:

In 88 the front and rear suspension geometry was changed. I don't think there is a huge handling difference between the two setups but could be wrong. The front suspension arms are different on the 87 & earlier cars to the 88+ cars. The wheel offset is different between the two suspension setups - that is why you need spacers to fit late C4 rims to an early C4. Not too sure on the basic types of suspension setup but I don't think the basic design was really any different - I don't think its a five link...
