Mine had the problem that it wouldn't work by using the switch but would work off the accelerator.
This led me to look at the micro-switch at the base of the gear shift. Replaced it for 14 bucks and now it works perfectly.

It might be working without you knowing it. Put the shifter in D and run up to about 50 MPH. Then put the shifter in OD. If it shifts then OD is OK. If it doesn't shift then you need to have a trans shop look at it.
