You don't have to remove the fuel rail but you do have to remove the plenum.
Remove the plenum, remove the gas cap (relieve pressure in the tank). Remove the safety torx screws holding the regulator cap and backup ring on. Remove the regulator cap, backup ring and diaphram. Replace with new and reassemble.

To answer your first question, yes that means your diaphram is torn so make sure whatever one you replace it with comes with a new one.

Sometimes. You really won't know until you get it off. Get a rubber mallet ready as they can be stuck on there pretty tight! Also there is a metal vacuum line that goes to the brake booster on the rear of the plenum, be aware of that.

Replace the gasket, always. Advance auto sells an "upper" gasket kit for it, just comes with the plenum, runner & throttlebody gaskets.

If it's a stock setup there will be a connector the MAT sensor in the plenum. Once you have the plenum loose raise the rear a little and unplug that sensor.

There are two rubber vacuum lines on the right of the TB and a hard line under the middle of the TB, use caution unplugging this as the hard lines become brittle with time and heat cycling. I prefer to separate the TB from the plenum before removing the plenum. Most likely you can reuse the gasket between plenum and TB.

Be aware that the gaskets between plenum and upper runners are "plugged into" the plenum with plastic divits. So try to keep these gaskets attached to the plenum when removing it.

When installing the Torx bolts into the plenum finger start all of them well into position before putting a wrench to them, to avoid cross-threading any. You may need to really "coax" the plenum to get all bolts started.
