Over the past year or so, on first start-up of the day usually dies almost immediately after starting. Well it graduated to twice and now the other day it took 3 full starts to stay running.
Fuel pressure is good at 45 psi and leak down takes about an half hour/45 minutes to zero psi.
Throttle body was recently rebuilt and cleaned
I have cut plugs
K & N freshly cleaned about 6 months ago. (2000 miles)
It "feels" like a fuel dellivery problem.
Recent timing chain replacement. (but it was a problem before that)
Start with something simple...change the fuel filter.
Sounds like you have a fuel pressure gauge. What happens when you try to start the car? Is pressure at 45? I run my 85 at 40 with the line to the AFPR disconnected and plugged. When you get in the car and turn the key "ON", do not start. The pressure should build and stabilize. Then start...what happens to pressure?
On a cold start if it quits, pull some plugs and see if they are wet. GM says you should see only 1 psi drop in 1/2 hour. You may have leaky injectors. Pull a plug wire and have someone crank a couple revs. Spark should easily be 1/2 inch. Weak spark can cause hard starts.

to me, your leakdown pressure numbers are not right. my 85 had hard starting problems and after lots of headbanging, I finally traced it to a ruptured fuel pulsator. you might want to consider this.
