It's on the airbox, driver side. It looks like a hole where air can get sucked into the motor without going through the filter. Did something used to be plugged in here? Should I cap this hole?

That's where the hose coming from an air pump (located in lower wheelwell) connects from. If the air pump is no longer there (you or some previous owner removed it), then I would cap that hole.
A hose for the Air Injection Reaction (AIR) system goes there. Cap it off since it's behind the air filter and the engine will suck unfiltered debris thru there.
I would cap it. Mine doesn't have that. What is that cover over the filter?
Not sure what you mean...the top of the filter is stock, except I cut off half of the rubber "thing" that is on top of the filter intake. You know, the whole cut lid mod. Got me like...35 hp
