I saw 1 that was custom, to fit in the drink holder - looked like a hockey puck with switches.
I tried to get the guy to sell it but he didn't want to split up the system he had 4 sale.

This is what I made I used the factory accessory plug under the center console for the ground and key on wire. Still need to add one more switch for the bottle heater. Right now its the line lock,ARM and purge.

I took an L-shaped piece of black plastic and mounted it by my right knee when I'm in the driver's seat, right at the front of the plastic cover that covers the underside of the dash. I drilled 4 holes in it, for nitrous switch, bottle heater switch, purge button, and torque converter unlock switch. I then used switches with little lights in the tip, all different colors. The whole thing is nearly invisible, unless you bend down a little when the driver's door is open you don't see it.

When I bought my 91 the bladders for the sport seats were popped. I had them fixed, and 48 hours later my wife popped them again. Instead of fixing them a second time, I decided to use the switches to control my nitrous system. The switch for the compressor was already momentary, so I used that for the purge, and the rest were replaced with small switches from Radio Shack:
