"P" keeps going off,not engaging !

"P" keeps going off,not engaging !
i think its misadjusted....
i adjusted the P,it was right ! it parked the car.
then later,i tried to put the "P" and i heard tack tack tack tack while i pushed my shifter to "P" as my car was moving forward,i went down and adjusted it,it was OK for a few parks,but then again,its loose again !
what i did wrong ? it keeps disengaging !
maybe i have to take the pan off and adjust it internally ?
