Default Lean exhaust code 44

I'm getting a lean exhaust code, but the car is running rich (smells, black smoke etc), so the computer thinks it is lean and is adding fuel. I've checked everything, replaced 02 sensor with AC unit, no change, can't find any vacuum leaks or anything. I'm thinking of replacing the intake gaskets anyway just to be sure. Anything else that could cause this? I have a PCMforless chip that eliminates EGR codes, but everything is still connected except for exhaust tube. Could this be causing problems?

Maybe an exhaust leak at the manifold/headers?

If it is an exhaust leak, it isn't a very big one. I replaced the IAC last summer, and put another new throttlebody gasket on, just in case. I may replace the collector gaskets with copper to see if that helps.

When I last had a lean code on my 86, I tracked it down to the tiny, pin-hole leak in the #5 header, right where the header tube made a downward turn. Maybe 3" from the exhaust port on the head.

On the previous occasion, my 02 sensor had died.

Was your car running rich when you got the code? Maybe I should replace the header gaskets too. It doesn't seem like a small leak would suck enough air to throw O2 sensor off that much.
