Default Setting idle w/ cam and TB?

My '87 has a mild cam and a 52mm throttle body

Idle surges when it's cold but it's nice and smooth when it's warmed up

I followed the idle setting procedure, no change.. what am i overlooking?

The idle rpm is controlled by the ECM which commands the pintle position in the IAC. You have to re-program the ECM in order to change it. All you're doing is messing up the minimum idle air.

I think that depends on the cam set-up and how it feels. I believe stock is around 550-650, might be a bit low I would probebly bump it up 100 rpm in the tune and set w/IAC cts. to around 40or so., and maybe even a tick more timing there as well.

How mild is the cam? Auto or manual transmission?

Normally when you install a cam you get a custom tune, did you? If not, might want to look into that.
