I called Tom at Carolina Clutch and he said the throwout bearing was correct, he said there's 2 styles and the other style wouldn't fit on the transmission.
He also said all the parts were the correct thickness. (although machining the flywheel sounds like a good option right now )

The new clutch disc was not labeled, but it only fits one way, there is no possible way to bolt the pressure plate up if it's installed backwards.

The slaves with wrong seals were only the stock style slaves, the replacement he got is a cast slave. And we already checked the pushrod lenghts, they are exactly the same, we even tried using each one.

The one thing I did notice was that the pivot ball had groves worn in it from the clutch fork. I think it is very possible that if the ball and fork are worn, it could very well cause this problem. We may have to change out both before we do anything else.

what is everyone thoughts on this?

I think you're to the point it's time to pull it apart and inspect the parts. I would still compare the old throwout bearing to the new. I'm going from memory, but it seems like the blue tag ZF used a different p/n than the black tag ZF and while it would fit, it wouldn't work. Just a guess on my part, but at this point I'd be checking everything.

Edit: I looked it up. The blue tag bearing will not fit over the black tag ZF guide tube. If you have the wrong bearing, it'll be obvious since there will be bearing and/or guide tube damage when you pull it apart.

Believe me the locktite did cause my problem. It was just enough to keep the clutch disc spinning as to prevent me from putting the transmission into gear. When I took it back apart, I could see it. I cleaned that up and did nothing else, and it fixed that problem. The clutch plate is pressed very hard to the flywheel. The very clean smooth surface of the flywheel and the locktite stuck just enough to keep it spinning.
