On my Deltran Battery Tender it means one of two things:
1) Reverse polarity connection to the battery.
2) The chargers safety timer has activated due to the battery not reaching its optimal voltage. This may indicate a defective battery. Take battery to be tested.

ALTERNATING RED & GREEN LIGHTS FLASHING (PLUS) This is abnormal and most likely indicates either that the battery is sulfated or that there is a poor electrical connection between the charger DC output and the battery posts.
Well I just had my battery load tested and it passed. After the test it recovered back to 11.8. But my alternator failed the bench test. No power out put. Just so you know, I went with Delco's OE rebuild. Delco has 3 types of rebuilds; their best is the OE which uses all GM parts and replaces the bearings, next is their Gold (professional) which uses quality replacement parts: and then there is there Silver (advantage) which uses value parts. Part stores usually don't carry the OEM rebuild. Rock auto has the OEM rebuild.
The Battery Tender 12V/5A charger i purchased showed a fault when I connected it in to the 12V outlet plug in the trunk. It would not charge the battery.
Deltran Technical Support told me the charger must be directly connected to the battery. I doubted the rationale since the wires from the outlet go to the battery.
I tried his suggestion and the charger works fine now.

Don't know what make or model you have but I found this:
RED & GREEN ALTERNATING LIGHTS: When the red and green lights alternate, then either the 72- Hour Safety Timer has been activated or the battery is defective. 6. Check the battery condition. It may indicate that a very large, good battery is not fully charged, or that a smaller size battery is defective.
