Header bolt backed out. Not sure how long it has been like this, but hasnt leaked.
93 LT1 DRM 1 3/4 SS headers, with Fel Pro gaskets. Cylinder #3.

I have a 92 Foxbody that one of the header bolts backs out. Only the furthest rear bolt on the passenger side. I have not lost it yet. I check and snug it back up periodically. Never had a leak because of it.
I thinking the header gaskets and headers have been there so long, its probably glued pretty darn good.. Headers are thick too..
Just weird after all this time, over 12 years.

I just screwed it back in, and a dab of loctice, high temp for measure.
It has time serts in there, not heli coils.. If that backed out, well, I would just pull the engine, and do some modifications...

I don't know. Never tried any of these things at the time. First one would make me wonder if it is more susceptible to sticking in the head. I have used the one I recommended.
I have the plain old ARP Header Bolts and they have never loosened in the 25+ years on my Hedman Headers. I torqued them to specs and never had to tighten any of them.
buy 5/16 id nordlock washers. drill them out with a die grinder to fit the header bolts. torqe them down and they will never loosen up. also for aluminum heads. headers and felpro gaskets its a good idea to retorque after a couple heat cycles. this is very common header bolts get loose. Ive used nordlock washers for years on headers and never had one come loose or leak, and its half the cost of stage 8 or other locking bolts. make sure to use the brown anti sieze on the header bolts
Found the problem. After putting the bracket into bolt to the air conditioner, I took the bracket off cause I was trying to troubleshoot vibration from the air conditioner. The bolt I used was longer than the time sert, so when I took it out it loosened the time sert.
i have a new time sert I will install soon.
Arp bolts are fine, the other ones after wel over 12 years, are still tight.
This was purely owner error.
There are some great header bolts suggested here.
I had issues with header bolts backing out about 40-years ago. I've been using high collar split lock washers (sold as header bolt lock washers) for many years and never had one back out. Just make sure the bolts you use engage far enough in the heads.

By far the best header gaskets I've run across (just recently) is the RemFlex (thick) graphite header gaskets (especially when there are imperfections in the flange). A little pricey but worth it for headers with sealing issues. https://www.summitracing.com/search/...ll-block-gen-i
