How often should lower ball joints be replaced or what's a test to show whether they're bad or not?

They are probably done just after 100K, or well on their way, although its not a super-critical part unless you autox.

Usually if theres any of the little nub sticking out, theyre still ok. Though I dont think thats always the case.

Could slightly worn ball joints affect an alignment?
Yes. If the ball joints are worn, they will allow the wheel to move around. If the ball joints are really bad, you won't be able to align the car.
