Does anyone have a diagram for replacing a Center High Mounted Tail Light (LED). Sometimes it works sometimes not. Any ideas on replacement would be great.

Have you checked it for a bad connection first?
could just need a little cleaning on the connector.

Remove two inner tail lights.
Reach in and with a 7mm socket, unbolt the 2 bolts on the bottom of the 3rd brake light.
Remove through tail light holes.
Reverse to remount.
Remove two inner tail lights.
Reach in and with a 7mm socket, unbolt the 2 bolts on the bottom of the 3rd brake light.
Remove through tail light holes.
Reverse to remount.

Also I took out the license plate, holder with the backup lights and removed the right license plate light, reached up and I was able to clean the connection, but I cant see were the screws were so thought a diagrame might help.
mine went dimmer and dimmer with age , replaced it on Ebay . not a cheap item from the "stealer ".
As far as "at times working and at times not " sounds more like a connector or electrical issue .check it sometimes when it is not working , remove connector and make sure you have voltage at the connector ( voltmeter ). if you have voltage at the connector when brake is depressed but the light does not work when connected , then I would replace the light . if the voltage is missing , I would check for other problems in the wiring before I would fork out the money for a new light .
I had the same problem with mine. I took the connection apart several times and cleaned it. Each time I did this it would work for a couple of days then go right back to the way it was. Finally I took out the entire LED light and found that one of the male prongs on the light was bent. Probably came from the factory like that. I was able to gently push it back into place and it has worked ever since..
