when I change the oil, I jack up one side front and put a rhino ramp facing forward and then I jack up the other side front and put a rhino ramp facing backward, THEN I jack up the back of the car at the center and install jackstands and leave the jack underneath. I hope the car never rolls because there's no way to chock on the ramps, but with the lips on the ramps opposing each other, I have a better than average chance of the car staying put!
The jack may have been 'pulling' the car slightly backwards as you were lifting it...and it just happened to be enough to get the car rolling off of the ramps.
Exact same thing happened to me, don't feel bad, as said before no one got hurt. One of my jack stands shot out the other one wedged between the rocker pannel and floor pan. Cost me $300 to have the rocker fixed. No more ramps for me, I just finished installing a 4 post Rotary Revolution Lift.
you had the big guy above smiling down on you here. just the fact that this rolled after you had the jack stands under it and not while lifting the rear end amazes me. one would think it would have happened with the floor jack and its wheels rather than with weight on a jack stand.
just a little advice, and dont take this wrong. DONT WORK ON YOUR CAR IN THE AIR AGAIN!!!!! it may not turn out this good next time. the car can be fixed. that same hole in you may not be able to be fixed. and based on your question about why while in park the front wheels where able to roll. PLEASE DONT WORK ON CARS IN THE AIR AGAIN.
the little bit of $$$ saved on an oil change aint worth what could have happened here.

The car can be fixed, but you could have been fulling that hole in the floor with your nose.
