A tuner will use a computer or hand-held device to optimize the air/fuel ratio, timing, etc. so you get the best HP the car can deliver based on the car's equipment. You can pick up some good HP by doing the tune if the tuner knows what he is doing. Prices for the tune generally run anywhere from $350 to around $500.

Not having many mods, I did the tune to eliminate detonation. I was extremely surpised to see the #'s. (BTW, I had a "street" tune, then later went to the dyno)
On my first tune after the mods I gained over 11 RWHP, but on the re-tune a couple of years later I gained an additional 10.7 RWHP, I had slightly different mods though, but still a nice surprise. If your stock or lightly modded I would go with a Diablo Predator. With mods go with the dyno tune~Well worth It
