What worked for me was to relax the small muscles (hands, forearms, etc.) at address and don't try to KILL it (i.e. ~85% power). When I would try to bomb the ball, I tighten up and lose yardage. Just a suggestion.

Without seeing your swing, I can't make any steadfast suggestions. BUT, I will say, make sure you have it teed high enough(over half of ball above top of driver), place 60% of weight on right foot(for a righty) in your setup, make a full shoulder turn(without trying to swing the club any further than normal), transfer weight to left side to start the downswing(do this by TURNING your hips, not SLIDING them), make sure to keep your head BEHIND the ball at impact, and make a full follow through by letting the clubs velocity bring you up and into your finish. And above all, RELAX! The club isn't going anywhere without you, so don't be in any hurry.
Any more problems, send me a video of your swing, and I'll see if I can break it down for you. Two swings from the back, and two from the side, minimum.
