different cords/ accessories and different ipods/iphones have different DC
basiclly the older accesories/ iphone/ipod run off a Firewire-based (+12VDC) conector
newer ipods run off a USB-based (+5VDC) connectors.
"Without any warnings to their millions of customers, Apple have changed the charging circuitry of their new devices so they only accept charging from USB-based (+5VDC) chargers. Earlier i-products also could charge through Firewire-based (+12VDC) chargers as seen in many car-installations and integrated iPod speaker systems. "
basiclly its either your trying to run a newer 5dc ipod/iphone through a 12 dc connector
or vise versa.
if it used to work and this is something new to you and it has always worked before then there is a few possible problems..
1. there is a problem with the cord, a cut or split or its just getting old.
2. the connector on the cord or the inside of the ipod/iphone is dirty and needs cleaning.
3. if you have jailbroken it, that would cause it to stop working sometimes.
hope this helps!

You can always get a 12v to 5v converter pass-through like the "iPod / iPhone Charging Adapter Converter (Black) - Convert 12V to 5V - Converts Old iPod Dock, iPod Alpine, Pioneer and Car Hook Ups to 5V Allowing You to Charge the New iPod and iPhones" by XTG Technology from Amazon.
Can you explain why a 12v to 5v pass-through won't work? My existing cable will charge 12v iPods but doesn't work with my 5v iPhone. By theory shouldn't the converter work?

you kinda answered it
you need a 5v charging cable
a 5v charging cable will charge a 12v ipod as well but a 12v will not charge the 5v
Apple is the one who changed this a couple years ago. When Apple changed this PAC changed the kits to allow for the 5v charging to come with the PXDP
anyone who upgraded an ipod or to an iphone who had older kits had to buy the new charging or ipod cable for the pxdp
