It depends on where the nail is. If it is a GY EMT you should take it to a Goodyear store and have it patched from the inside, GY won't patch the tire if the nail is < 1" from the edge. This way you will lose only one letter in speed rating. Personally, I do not trust plugs.
Also, EMT's can only be patched twice before GY deems the tire unuseable.
I have heard that other brands of runflats cannot be patched.

Short answer: Yes, you can patch it.. but in order to maintain the speed rating and runflat capability of the tire, it must be an umbrella-type (plug and patch) applied from the inside, with proper preparation.
