I reach in from the fender well with a long nose vise grip and grab the shaft flats just above the nut. I could then spin the nut pretty far off with an open end wrench before it tops out into the vise grip. I then remove the vise grip and wrench the nut off the rest of the way. Once you loosen the nut several turns (vise grip on) the final turns don't have enough resistance to spin the shaft.

If the thing is really rusted or frozen on to the shaft, and you will not be reusing the shock, you can cut the plastic dust cover off the shock and just grab the shaft, right above where it comes out of the shock, with a pair of vice grips. I ended up having to do this with one of mine.
