Thank goodness no one was hurt.
I have a Eagle along with 7 of club members at the time we purchased they were being made in Texas and now after last year they also are produceed in China.
Eagle stated that you did not have to anchored down if you did not want to but in turn they sent along with the unit 6" long sleeve anchor bolts to do so. I did try driving the vett onto without anchoring down an that is crazy since the lifts wants to move.
Anyone that does not have anchored down you should do so.
My two cents

Its hard for me to believe ANYONE buying a lift does NOT bolt it into the concrete..
I whould think THAT would be the FIRST item on the agenda,
concrete anchors & Hilti or other brand hammer drill..

four post
I have two four post AutoLifters that I bought in the mid 90's. They allow me to have 4 Corvettes in a two car garage. No problems with my use. I assembled them with help from no one. I see no reason for any problems in the future.

I have a Direct Lift and because of the tension cables in the foundation (earthquake requirement) I cannot bolt the lift to the floor. Being So Cal, I was very concerned because of the earthquakes we have from time to time. So I drilled a hole in each post, just above the lock point I keep the car at. I then raised the car ran the bolt through the hole in the list post, then let the lift onto the bolt. No way that thing is coming down. May tip over but the bolts have to shear before the car comes down. Just added insurance!!

I live in Canada and noboby on the USA side has mentioned BEND-Pak , in Cal. I bought one about 5 yrs ago and stayed away from all the low tech ones, I only had one issue with mine and it was a busted Hydraulic hose . After it sprayed oil every where, the lift just settled down to the next locks, and that was it , a big mess of oil but the vette stayed up there. Ron
Can't see the pics but glad nobody was hurt.

It may be a 4 year old post, but it doesn't hurt to remind everyone who has any type of lift of what can happen and the importance of continued scrutiny and maintenance of these lifts.
