I don't have a diagram handy but can explain its routing on my '99. From the rear of the driver's side valve cover, it runs behind the intake to a "Y" connection. That "Y" connection allows a short tube from the passenger side valve cover to join with it and the output of the "Y" then goes along the passenger fuel rail to the PCV valve. Of course the output of the PCV goes to the intake. Two things that I've seen go wrong with mine. 1) The PCV output rubber hose to the intake can develop a hole on the bottom side of it out of sight. 2) The "Y" connection behind the intake can desolve and come apart without any obvious signs. Add a 3rd item for good measure its a real pain in the posterior to replace it.
