Exhaust hanger malfunction?

Exhaust hanger malfunction?
I crawled under the car while it idled in the carport area and immediately noticed the DS rear exhaust hanger holding the muffler and Ti tips was bouncing away from its mount and the nuts that secured the bolts were gone. The tips and muffler were just swaying freely (which accounts for the increased vibration feel at idle) and the banging scraping sound.
So is this a common issue or did someone forget to tighten the hanger nuts and they just worked their way off?
Alas I am still a little dismayed that both nuts just backed off and let go while on the freeway... Kind of odd IMHO...
