Default Clutch Woes

Im not sure where to go with this one, not sure if its the slave or the clutch or anything else related.

Problem: As I take off from a complete stop the car has begun to require me to ride the clutch out more and more to the point where i hear a squeal (sometimes), the car also feels motionless once ive finally released the pedal (sometimes) and i have to give it some gas as to where the car begins to move. All of this is causing my rear-end to hop around all over the place, I was wondering if anyone else could help identify these problems and point me in the right direction, thanks.

Hmm, have you removed the inspection cover to have a look at the clutch? I don't know what to think.

Those are some different problems. Do you notice the clutch slipping at all? A good way to test this is to get on the highway and put the car in 5th or 6th gear, give it some gas and see if the RPMs shoot up or the clutch grabs.
